RULE 54-101 - FORMS FOR NATIONAL INSTRUMENT 54-101

PART 1 DEFINITIONS

1.1 Definitions - Every term used in this Rule that is defined or interpreted in National Instrument 54-101 (the "Instrument") has the meaning ascribed to it in theInstrument.

PART 2 DESIGNATION OF FORMS

2.1 Designation of Forms

(1) The required form of explanation to clients and client response card under the Instrument is Form 54-101F1.

(2) The required form of request for beneficial ownership information under the Instrument is Form 54-101F2.

(3) The required form of omnibus proxy for a depositary under the Instrument is Form 54-101F3.

(4) The required form of omnibus proxy for an intermediary under the Instrument is Form 54-101F4.

(5) The required form of a non-objecting beneficial owner list under the Instrument is Form 54-101F5.

(6) The required form of request for voting instructions made by a reporting issuer under the Instrument is Form 54- 101F6.

(7) The required form of request for voting instructions made by an intermediary under the Instrument is Form 54- 101F7.

(8) The required form of written undertaking concerning the use of NOBO lists under the Instrument is Form 54-101F8.
